Registration Requirements
- School application (front and back).
- HRS #3040 medical form obtained from your child's doctor. This form certifies that your child has received the immunizations required by the State of Florida. The physical examination is valid for two years. This form must be in our office prior to the first day of school.
- Registration for our church families and current school families is accepted the first week of March, for the following Fall. All others will be accepted beginning the second week of March and thereafter.
